Care Coordination Case Manager, PT #147-18


Job Summary:
Under the supervision of the Care Coordination Manager, this position as a registered nurse or licensed practical nurse with experience in the hospital setting is responsible for ensuring that all assigned patients meet admission requirements for acute care; have appropriate DRG/length of stay identified and continued stay reviews. The job also includes Discharge Planning, Social Services, Case Management and other activities to ensure that specific patient outcomes are achieved.


  • R. N. or L. P. N. currently licensed in the State of Indiana
  • CPR Certification
  • Annual PPD
  • Annual fire/safety training
  • Corporate Compliance


  • Knowledge of current medical terminology and procedures.
  • Excellent communications skills.
  • PRO and insurance company standards and criteria.
  • Ability to establish smooth working relationship with physicians, their office staff, and

ancillary hospital staff.


  • Ability to abstract medical chart contents.
  • Computer literacy for P. C. and hospital information system.
  • Ability to operate hospital phone system.
  • Knowledge and ability to operate general office equipment.


  • LPN or RN Graduate of School of Nursing.
  • Three years physician office, emergency room or hospital nursing area experience.
  • Knowledge of Healthcare financial systems.


Demonstrates knowledge, skills, and behaviors appropriate to the populations (s) served, and based on specific criteria and/or age specific considerations, as follows:

  • Age appropriate communication skills for adult and geriatric patients.
  • Knowledge of age appropriate community resources for adult and geriatric patients.
  • Ability to assess and interpret age specific data.
  • Ability to include family and/or significant other in discussions with adult or geriatric patients.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
